The entire Cox IP block is listed.

the header (unless I use web mail, which my server does support, but I
don't like using web mail).

Cox blocks outbound ipv4 port 25 from their network anyway. Do you have
business class service?
Most big cable and dsl providers disallow outbound on smtp nowdays due
to spambots.
Lots of places are dropping email that doesn't reverse dns to a valid
mailserver. Make sure if you are on cox business that they give you
reverse dns.
