There is only a single ethernet port on the DSL Extreme modem, connection by RJ-45 jack to an unshielded twisted pair cable. The phone connection is via RJ-11 jack to a supplied CAT-3 cable.

Searching the fine print on the AT&T web site I see that the $50 DSL
modem that comes as part of their standard deal has a single output
port.  For $20 extra you could get one with a 4-port switch built in.

DSL Extreme supplies the modem at no additional charge, plus two of the filters for your regular phones. I got a 8-port gigabit switch on sale at Fry's for $25 a while back.
