I bet that NeXTstep could do it too. OS X uses PDF as its native
display language, NeXTstep used display PostScript.
What happens if you print from simplegtd to a regular 8.5x11 sheet?
Where on the page is the 3x5 card image?
Can you print to a file? This should give you a PostScript file that
can be edited by the text editor of your choice to translate and
rotate as needed to fit on the 8.5x11 sheet in the place that the 3x5
card would be when it is run through the printer. gv (GhostView) is a
great tool for looking at the results without actually printing them.
Once you figure out what transformation is needed, you can use the
scripting language of your choice to do it automatically. Sounds like
a job for sed(1).
