This is specified in the PC99 System Design Guide, promulgated by Intel and Microsoft years ago. It's now being sold in hard copy, but is still available as a PDF here:

This used to be widely available on the Internet and apparently has been taken down from most websites, probably to support book sales.

The color coding is described in section 3.18.3, and some of the icons appear in Appendix A.
