Being the amateur writers we are, we occasionally forget a word or muddy a 
phrase. Yes it would be great if everyone wrote like Charles Dickens.  But 
since we don't,   we need to try to understand the point being made.
 I took the sentence to mean that the throttle was to be left in the almost 
full 'out' position or in other words: just crack the throttle.
