Here's a few thoughts


TD1232 master slaves are notorious for weird problems. What versions
are the ksu's?

A short on an extension can take down a group of extensions, usually 4
or 8 on the same circuit card (on the same amphenol connector).

The phone company typically doesn't know what they are talking about
when it comes to phone systems. So I would ignore their comments,
unless you know differently.

Usual problem are related to loose connections, cords or connectors.
The mod connectors in the phones can become loose (bad solder
connections)
If you have swapped all - line cords, AND the phone set itself- good,
that rules them out.
Make sure you really did swap things yourself and not that someone
said they had done it when you weren't there. Mark things so you know
which phone it was.
Check the jacks on the wall. The punch blocks could have a problem...
If necessary, run a cord from another location, even from the ksu if
it's easy enough, all the way to the phone. Give her a different
extension, you can program it to be the same ext number if needed.

The amphenol connector can get loose in the cabinet. If it starts to
pull off it could disconnect one or two extensions. (usually the first
ones on that block - 101, 109, 117, 125, 133, 141, etc.

AND- check for patterns and clues in the way she works, the times of
the problems, etc.

In one company, I had presumaby a short on an unused extension analog
pair. Every day around 4:30 give or take the system would reboot.
They were using existing wiring in their building (so I didn't know
where it all may be going). I took off ALL unused analog pairs and the
problem went away. (That was after weeks of troubleshooting too).

Another case (not like yours, but lesson learned in it), the girl's
speakerphone always went bad. We swapped phones, swapped circuit
cards, swapped cords, everything. Finally I swapped her phone one more
time and I stayed to have a cup of coffee and chat after we put the
new phone in.  After about 1/2 hour and lot of test calls she was
satisified that it was working (as it had before...) so she then
proceeded to put all of her post-it notes and scotch-tape notes all
over the phone INCLUDING right over the microphone hole- causing the
"speakerphone to malfunction"!
