I copied Alice's description below because it will make my explanation easier
:-)   I have found a way that completely prevents bunny ears but doesn't
require carefully repositioning the pin (which would drive me crazy and be
slow).  Pin the bottom thread in the target pin-hole. Lift up the second
thread high enough and just far enough into flipping it around to be able
to reach underneath it with the forefinger of your other hand, and pull the
top of the picot pin very slightly back and out with the nail of that
forefinger. Now finish looping the second thread around, sliding it down your
finger so it just goes around that pin and no others.  Voila!

picot has two threads twisted, then the bottom thread is pinned in place with
pin in fingers, but the top thread is flipped around the pin. The cure to not
catching a second pin when 'flipping' is simple. When looping the first pin,
pin it an inch outside your pattern. Flip the second thread around (no other
pins near it). Then carefully lift the pin with the threads around it and move
it gently into it's proper place. Keep the tension mild until final pinning,
then tension the threads, make the final twist(s). Wow!! Perfect picot and NO
extra pins.
