I was finally able to get a stable install again, not sure exactly what the issue was. I set both the pcic and cardbus debug level to 1 in /etc/system and rebooted. Attached is the full messages file. Please note that I booted with the card not inserted, and that when I inserted the card there were no additional messages logged.
