My two-pence (or cents) worth. 1. Try and get the index changed to show the name(s) on the original source document, but always remember the source is the document NOT the index. 2. Record the name as given on the original source document - even if you 'know' it is incorrect. 3. Make a note of the discrepancy under the detail of the source.
