Hi Dick,
One suggestion would be to include in the "Detail" the name of the individual to whom the certificate applies. The reason I do that is because this certificate will probably be attached to several facts; date of birth, name and age of parents, etc. Later on when you find this source attached to one of the parents, you won't have to go digging to find just which of their children (or someone else) did it come from. It will be right on the report in front of you.
