You've made one important decision - you know you want to keep the 4 field
format.

Coleen's suggestion is excellent - it also allows for use of the same format
for Ireland, Scotland, and Ireland  (all part of the United Kingdom).

For other countries you will need to address them on a case by case basis -
seems to me that the City, County/Parrish is fairly well established as a
naming convention for most Countries.  The addition of a State naming seems
to apply to countries with very large geographic areas.  I think the key is
to establish a standard to be used for locations in countries where only
three makes sense - that way you will know to always leave the X space
blank.  I would use 1,2, X, 4.  You may prefer 1,X,3,4 - whichever way you
chose is fine - just be consistent!
