What it's telling you is that there are two separate political entities:
Richmond (the city) and Richmond County. If you look at a map of
Virginia counties, you'll see a "hole" in the middle of some of them,
where an independent city is located. The county does not have
jurisdiction within the city boundaries. 

It's not really a function of a city and county sharing the same name.
One example is Frederick County, at the northern tip of the state. The
City of Winchester is the "hole" in the county donut. :)
