I have the same problem.
On the report when the "Report Options -> Include -> Alternate Names" box is not checked:
5. William Joseph SCHMALZ was born on 22 Jan 1899 in Cleveland, Cuyahoga, Ohio, USA.
When the "Include Alternate Names" box is checked:
5. William Joseph SCHMALZ was born on 22 Jan 1899 in Cleveland, Cuyahoga, Ohio, USA. Another name for William is William Joseph SMALL.
I don't see an entry for the alternate name, "William Joseph SMALL", in the index. Is there something else I'm missing? I'd like both names in the index. (Tried it on Descendant Book and Pedigree Web Pages)

I did do this but the alternate name does not show up in the index.
