There's a limit to the amount of information you can get into a file name.
I begin the name of the photo with a code for the source and a number - the order in which I've scanned. I like to know who holds the original. I also like to get on with the job when scanning as tracking down who and when can take some time and comparison with other photos.
I add name and date if known at the time or come back and add it later.
I add further information in IPTC captions
That's how the photos are archived.
I sort my working copies into relevant folders.
How many folders depends on how many photos I have for that family. eg for my own family each sibling has a folder and each of their children have folders, however for more distant family when I have fewer photos I have folders such as "Harris English"
The disadvantage of my naming system is that photos don't sort automatically for a slideshow etc so for some I've added a folder code and number to the filename. For simple batch renaming like this I use Irfanview where you can order the photos before batch renaming.
The problem with being reliant on the album system or description within some photo software is that the field is changing so rapidly and you need a system that converts easily to other software. I've tried a number of different programs.
I'm using IPTC captions as a number of different programs read that information and perhaps more will as it seems the most used standard. The new Picasa from Google uses IPTC captions as does Irfanview, BreezeBrowser ...
