So, perhaps a search looking for individuals with blank Alt Surname should
return those plus individuals without Alt Surname fields, two different
things from a database structure perspective but the same thing from a
user's data perspective.

That is exactly the case, If you have not entered either an alt given name
or an alternate surname the field does not exist. (This is the way
relational data bases work, the link between a person and an alternate
name is in a separate table and only people with alternate names have
entires in that table. That saves storage space since only those people
with alt. names use the storage apace for an alt name and that is how you
can enter more than one alternate name for the same person, it is just an
extra entry in the alternate name table.) That is why searching for alt
names only gives you the individuals that have one or the other.
To get the 80,000 who have no alternate name. Use a search for alt given
name = wildcard OR alt surname = wildcard. this should give you everyone
in your file with an alt. name entry.
Tag everyone in the search list with a tag you know you have not used (say
6) or first use the advanced tagging to clear a tag then tag everyone in
the search list with that tag.
Repeat your search and this time look individuals where the tag you used
earlier is untagged. Now all the people without an alternate name will be
returned.
