Here are some examples - use them if they fit your way of thinking:

King Henry II "Curtmantle" Plantagenet

Surname:  Plantagenet
Given Name:  Henry II
Title Suffix:  Duke of Lancaster
Title Prefix:  King


King Edward I "Longshanks" Plantagenet

Surname:  Plantagenet
Given Name:  Edward I
Title Suffix:  King of England
Title Prefix:


Princess Leonor of Castile and Leon

Surname:  Alfonsez
Given: Leonor
Title Suffix:  Princess of Castile-León


Anyway, It is the way that I find useful.
Others probably have different versions.
