So, as we were considering splitting of a separate Object menu, the detriment
of having too many top level menus came up. And since Forms Controls are a
rather niche functionality - used by few of our users, and not frequently - the
idea was put forward to replace the Forms top-level menu with the split-off
part of Format. That way, the overall number of top-level menus remains the
same (although personally I believe it may increase in the future); and the
position of all existing menus, other than Forms that gets scrapped, is
maintained.

Then the question is "what to do with the form controls?"

On the one hand - I mentioned these are also "objects" - not exactly like
circles and stars and rectangles, but they do float, they have a bounding box,
wrapping and position etc. So in theory, they could be a submenu on an Object
menu. But - instead, it is probably better to make them to a submenu of Insert: 

Insert
   etc.
   Chart...
   Media        >
   OLE Object   >
   Shape        >
   Form Control >
   etc.

fits right in.
