" " [blank] is shown for headings in levels 2-9 in the index (i.e., their headings do not have numbers), and "evaluate up to..." is greater than 1. For example, if "evaluate up to" is 2 for a level 3 entry, then rather than showing the first level (which has a number), and nothing for the second level (which has no numbering was assigned), it shows "blank" (but if Evaluate up to is 1, then shows the first level).
