Works for me (TM), using the first patch you highlighted below.
Granted I'm using a <cough>intosh and used the existing benchmark device 
custom descriptor stack (I wish I had the time to try the new Microchip 
stack but I really don't), but it doesn't look like the issue is with 
libusbx.

Do you have the same issue for Windows and Linux, or not?

For what is worth, I produced a trace on Windows yesterday, and found 
nothing out of the ordinary with regards to the requests we send to the 
device. 0xC0 and 0xC1 were in their expected place, and even the wIndex 
looked fine to.

At this stage, if you see any issue, it'd be nice to have a trace to 
find out if the problem is really with our requests, or the firmware. 
Especially, given that the post from which this whole issue stems was 
someone reporting they had all the trouble in the world getting the new 
Microchip stack to work, I'm very reluctant to look further into it 
unless we have hard evidence that libusbx is actually doing something 
wrong. Moreover, I'm not planning to inspect all the changes that were 
made to core and OS-X since the libusb-pbatard branch (since I expect 
the relevant xusb code to be the same now) to figure out why old 
obsolete code seems to work for a platform that really wasn't the prime 
target for the development occurring on that branch.

I have now pushed the patch reversal to mainline, and closed #96 since 
by all account we should be sending 0xC1 as expected, and at least with 
the device I tested against, I have yet to see an issue be it on 
Windows, Linux or OS X.
