Reverting the previous patch with commit
4c5a265cf972dcf3d4482fcb03cfc4f7bbba5b01
clears the regression.
While we are thinking about this, we can test the patch to remove dots
per-voice

but it will wait for version 2.19.
I think Gould must mean staff-positions when she writes "stave-spaces" so
that a chord with highest note b' can have a dot in the c''-space, but a
chord which highest note a' may not. I made a parameter 'dots-limit' to
control how far the dots may extend beyond a chord, so we can adjust
according to our interpretation.
