Interesting... I totally missed this !

You're right, in my original file, I somehow missed adding the "4", but since 
it was the default value, i never noticed it.

This also explains why it continued to work when I cut and pasted it into a new 
file for troubleshooting.

Notice that the first note in the uppermost voice is dis4., whereas you need 
dis4. I suspect you did not assign an explicit duration to this note so the 
last duration of the preceding piece is used. When no music precedes a note, 
the default duration is a quarter note.
