Someone can correct me if I'm wrong, but I think you'll need to build lilypond from source. You'll want to have a look in the program usage about building lilypond from source, and then there is a thread in the developers list on using the binary wrapper once you have the program built. I haven't done this myself, but those are probably the best places to start.

Yes and no. Yes it is possible to use the program, even with the menu, it's a bit complicated. I would suggest going first to http://lilypond.org/web/install/ and click on the link for osx 10.5 for instructions on how to get the program working.
