I have the flag <format config:type="boolean">true</format> for each of my
disks, but it seems that my 0101 disk is not being formatted.  After going
through the AutoYast UI, and rebooting the z/VM virtual machine, I see the
following on bootup:

dasd(eckd): 0.0.0101: (4kB blks): 2403360kB at 48kB/trk compatible disk
layout
 dasdb:VOL1/  0X0101: dasdb1

How do I fix this?

The entry you want (one for each DASD volume) is:
<format config:type="boolean">true</format>
