The tools have changed over time.  I don't remember what is in rhel 5.4...
 Have you tried to use san_disc or lsluns on the failing linux system?
These are tools to explore what you can see out in the fabric.

Zoning comes in a few flavors.  There is port zoning where you designate
switch ports.  There is also world wide name zoning where you designate
wwpns.  Port zoning usually requires less effort to setup, but doesn't
provide as must isolation.  Which one are you using?

Are you using NPIV?  Are these linux systems guests or lpars?  With NPIV
the wwpn for devno 0.0.dc00 will be different in different lpars.  The
wwpn for devno 0.0.dc00 will always be different for each CEC regardless
of the NPIV settings.
