My diald connections were working perfectly, then we did an upgrade on the
ISP-side modem firmware.  Now, I will often get these errors.  When this
error does show up, ppp is killed (phone line reset), then it dials back
into the server.

I have a message into the sysadmin of the ISP as I beleave this to be a
problem with their modems.
