My fault sorry. 

No service.switch file, no smarthost (direct delivery) and deliverymode
background. Everything works perfectly now.

No need. By default the startup script does this on redhat. Nothing more
required. I start sendmail in ip-up.local. So no problems.

/usr/sbin/sendmail -q -v 
/usr/bin/fetchmail -e 15 -t 60 -v -a -f /etc/fetchmailrc -L
/var/log/fetchmail.log

is already present in /etc/ppp/ip-up.local.
