While making forms for ISO9000, I faced this problem and finally solved
it by making all forms using spreadsheets. The format was locked with
a password (known only to the MR), and the user could only fill the form, 
but could not change the format. But this was done in MS Office (excel). 
I tried doing the same in OO, but was not successful. 
I'll checkup exactly what I had done when I get to office tomorrow, 
and will let you know.
