Forget the vanilla qmail - see if you have a package for netqmail-1.05 or download it off www.qmail.org. There is a combined tls and auth patch for it that works just fine. netqmail is a collection of "need to patch these!" patches for qmail along with the vanilla qmail source and a shell script to build the stuff.
