It means that someone(in this case 216.252.170.193) tried to update your dns database, with his own name-->ip entry.
This is a common occurance if win2k is being used by the particular person in question since win2k by default tries to register its dns name on the server, and you have to specifically tell it not to.
The fact that it says its unapproved means that the client in question failed. This is usually good, unless you wanted to allow the client to do this(not very wise).
