Very strange, it all seems to point to a disk problem, have you checked 
that the cyls/heads etc, are correct, the fdisk section has a field showing
disk geometry, check that against thwe drive documentaion, possably your
BIOS has detected it incorrectly, this can and does happen with 'award'
bios'es.

Does the mount failed tell you which device is failing ???.

When i tryed to install 5.0 on my new ST34321A, 4103MB i noticed that my
BIOS had detected the disk wrongly, i then had the same sort or problem(s)
you are having.
I turned of the autodetect and enterd it manualy into the bios after that i
got over the problem.

Even fdisk formatted the disk seemingly as 4 gb, with the wrong settings,
but as soon as i tryed to create the filesystems the problems started.

This leads me to suspect you have a simalar problem.
Of couse i could be wrong.
