1. Yes, but I don't recall its name or location.

2. Depends. If you're doing it from Linux, any of these will work (replace
"somepath" with the actual path to the image):

        A. dd if=/somepath/rescue.img of=/dev/fd0

        B. cat </somepath/rescue.img >/dev/fd0

        C. cp /somepath/rescue.img /dev/fd0

If you are doing it from DOS or Windows, use the DOS program rawrite in a
DOS window.
