I'm a relative newbie to the programming world, so what I find
invaluable may be exceedingly basic to the more experienced
coder. Disclaimer aside, however, the grad dip course I'm doing
in computing (at the University of Canberra) uses Deitel & Deitel's
"C++ How to Program" as their standard text. It's not aimed at
any particular operating environment and it starts at the very 
beginning. It's a text book with lots of exercises. I think it's
great. I've got the 2nd edition, which covers the ANSI/ISO C++ 
draft standard. I'm not sure if a third edition has been published
yet.

I also have O'Reilly's "Practical C++ Programming". I haven't
used this book as much as Deitel &Deitel as it's aimed at a higher
audience. Having said this, the book does spend some time 
discussing compilers and IDEs under different systems 
including UNIX, MS-DOS and Windows. It tends to focus on
the MS-DOS environment, but UNIX commands, tools, procedures,
etc are also covered.

Both these books were readily available from the University 
Cooperative Bookstore (note they were each a little on the pricey 
side ~$70, fairly typical for computer texts here though).
