Now i dont know what the error is, however i do see many differenaces
compared to what i see at bootime from my SCSI adaptor and disk.
I see;

Now i belive you should also see something like the above.
However i am talking linux, could it be that your error is coming from the
bios because you have not setup the scsi bios correctly.??
I think its better to wait and see if you can shed some more light on this,
before i go futher with explainging howto do it under linux.
