OK,

Here I am again with the same problem I had before.
I have a new built server running RH5.2, fresh install.
To recap,

Hardware:
AcerOpen AX6B MB
Intel Pent2 350Mhz
AcerOpen PT75 S3Verge VC
Adaptec AHA-2940UW
2x Quantum 4.5 SCSI3 HDD
HP 4Ti SCSI2 Tape Drive
Toshiba 40x SCSI2 CDROM

The SCSI chain is as follows:

All internal.

The card is set to LowOFF/HighON per the guide instead of AUTO.

Again here is the problem:
Everytime I try to access the CDROM it hangs then the card tries to reset.
I have done all the things that were suggested but they all boiled down to
one thing "properly terminate you cables". Well I have done this (I think,
I'm still getting this SCSI s**t strait) but the problem remains.

The error I get is the same as before:
"scsi : aborting command due to timeout : pid xxxx, scsi0, channel 0, id 5,
lun 0 Read" or something like that. Then I have to reset the box or it will
get caught in a reseting loop.

Now (as said before) this system was setup using a IDE CDROM originally and
later the SCSI CDROM was put in. At start-up though the CDROM is labeled
"sr0" instead of "scd0". There is no "sr0" listed in dev/ but I was told
that sr0 and scd0 are the same (Doug Ledford) but I'm still stuck.

Can ANYONE help me find an answer for this?
When I decided to go this route I did not believe it was going to take near
THREE WEEKS to get an answer. I guess that because of my experiences to date
(we are using Linux an all servers save one) I have come to expect Linux to
work.
