I believe it's an INI-940 chipset, which is driven by the INI-9100UW driver.

You need the modified boot disks on http://www.initio.com. This will allow
you to install on a SCSI card with an Initio chipset. The drivers are
present in 2.2.1 and 2.0.37-preX. After you get the modified boot disks,
follow the instructions in that package.

I doubt that's the problem. You're just not using a bootdisk that can find
the driver. The kernel installed with RH 5.0 won't be able to find it
either, so you'll have to roll your own kernel.
I'll look into what's needed to create the customised boot disks for
RH 5.X, and post them on my homepage. I'll announce those on this list.
