I have:

  Bus  0, device  14, function  0:
    SCSI storage controller: NCR 53c895 (rev 1).
      Medium devsel.  IRQ 11.  Master Capable.  Latency=64.  Min Gnt=30.Max Lat=64.
      I/O at 0xdc00 [0xdc01].
      Non-prefetchable 32 bit memory at 0xdfffef00 [0xdfffef00].
      Non-prefetchable 32 bit memory at 0xdfffd000 [0xdfffd000].

and got ~same behavior when I connected an external tape drive, I
tested tree cables without resolving it.

I had an extra internal terminator so I attached an extra cable after
the drive and then the internal terminator, and now it works without
any problems.

So I would guess you have bad cable/terminator in some combination.
