Note the errno of 32 above. This appears to happen on an ioctl(USBDEVFS_SUBMITURB) of the device with a USBDEVFS_URB_TYPE_BULK type.
Huh. Well, if it were control, that might be caused by one of the UHCI bugfixes that aren't yet merged. It caused control stalls to happen in cases they shouldn't have. I don't think there was a bulk analogue.

No bright ideas beyond trying on non-UHCI hardware, sorry.
That would previously have been "control/bulk timeout", giving no very helpful for anyone with multiple devices...) Did you get that older message before?
Only when running "modem_run" which uploads the firmware, not when actually bringing up pppd.
I'm guessing that's when you saw it here too. The change which caused that message to change would not affect the usbfs async submit paths (USBDEVFS_SUBMITURB).
