Thanks. A few questions:

Do you need the US_SC_SCSI and US_PR_BULK, or did you just copy those
from another entry? Have you tried US_SC_DEVICE and US_PR_DEVICE?

Also, I'm not sure where you got the bcdDevice numbers for your entry...
0000-1020 certianly contains 0100, but it's kind of a random range.
