I am embarrassed. There WAS an external USB hub being used. I must have traced the wrong cable earlier. I moved the cable to a port on the back of the computer and rebooted.
That first reboot, /dev/video0 didn't get created. After an unload, modprobe, it did. Now the camera works with luvcview and cheese, with many more options for format, resolution, and frame rate than it was offering before.
