You can get both pieces of information with lspci.  Run 'lspci -vv' and
find the stanza about your video card, and paste that output. i.e.:

$ lspci -vv
...

This shows us  what video card, and what driver the kernel's using.
Also post the output of 'uname -a', since that will give us contextual
information about the version of the video driver.
