Anyway, anyone know about using a PDF document instead of a web form?

Not sure if a new Adobe feature with version 7 or 3rd party plugin, but the 
idea is to use a PDF document instead of a web form..

Not sure if it goes into a database, email or gets turned into a PDF
document.

Anyway, I'd prefer going online form to PDF document but I have a client 
wants to use PDF as the input form..

Any ideas, experiences?
