I have made an existing pdf into a form.
One needs acrobat to do this. (meaning not reader.)
HR likes it since they need signed documents. 
I can print it off and take home to fill out or fill on the blanks
like a html form then print it off. that way every body can read it.
(my writting is, like my english, terrible.
