The VOIP phones at my remote locations (VPN with IPSec) work well with
our PBX, which is asterisk-based.  We have had this issue previously
where the voice did not work, but I do not recall how it resolved
itself.  I think it involved rebooting the pfsense at the remote site.

Lately I have also noticed that my VOIP phone service at home gets
disconnected and the state table on pfSense shows "ENABLED:NO_TRAFFIC"
and "NO_TRAFFIC:ENABLED" as the two states for the SIP udp ports on
the adapter.  Killing those state entries allows the device to
re-register and the voice works again.  It re-registers every 60
seconds so I have no idea how those states get messed up like that.
