I pushed the changes into branch named LOG4PHP-200:

For me this solves the issue completely, and it simplifies the code too.
Can you please try it out?
The only "issue" left is when the superglobal is not set in variables_order. In
this case the substitution will not work, and no error will be reported. I
think we can solve this by adding a note to the documentation.
