Because, as is the first rule in communications:

"The telco is not your friend."

Well, it usually takes some form of prior experience to throw your 2 cents 
into a thread such as this, and most times, communications service providers 
is where typical posters would have background.

This is true - but they are still not your friend.

That some of your ideas go back to the drawing boards at the telcos, well, 
yep!

The scale of a telco operation is very very big - most times shadowing the 
small end user's typical requirements. Some telcos have remodelled their 
internal structures to cope with the way of the world as it is... 

... landline-based telco providers are fast losing voice minute business to 
growing GSM providers. Why? Because the new customers are coming on, and no, 
they aren't corporate Uganda, but the teenagers who ask mum and dad for a 
cell phone, or the early campusers who need to communicate while in a 
lecture; and the trend, mobility...

If a telco can make their service more granular, it's better for them. Large 
scale operations are great, but aren't everything.
