OK, I got it, but the install instructions are a bit lacking.

I figure the chances of Joe User getting it installed are about nil.
It is small enough that it could easily be set up with sane defaults and
included in the LyX tar ball. Specifically, the install instructions say
nothing about moving html2latex into your bin search path and editing to
point to where the tag files are (which also have to be edited).
