There was a bug in pdflatex with including PDF files:
I think that the problem only occurred with PDF files generated from eps
using epstopdf, and the eps file was generated from a bitmap image (example
platypus.eps).
However, this bug had been fix.
Furthermore, if you have a bitmap image you can insert it to the lyx file,
and then you won't have a problem when using pdflatex.

No, conversion to pdf is done in one step using epstopdf.

For pdflatex:
jpg,png -> inserted without conversion
eps -> pdf
everything else -> png
