mathed naming

I plan to do some cosmetic changes to mathed/*.

(1) Rename MathSqrtInset to InsetSqrt (in line with insets/*)
(2) Rename the files into InsetSqrt.{h,C}
(3) put everything into namespace lyx

Objections?
