On my copy of TLC2, pp 235-238 on in Chapter 4, "The Layout of the Page" and 239 begins Chapter 5, "Tabular Material".
Higher Mathematics is Chapter 8 and 8.2.3 (a single equation on multiple lines: no alignment) has the LaTeX code on page 471. I guess I'll enter the entire equation in an ERT since I apparently don't know how to properly manipulate the LyX math mode.
