I did try a simple document and there was no index. However, your suggestion helped me find the problem. In case others run into it, I'll list what I did here. Under the Tools>Preferences>Output>LaTeX I changed the setting from texindy to makeindex. Then I made the same change in the document settings menu. After I reconfigured and restarted Lyx, I had an index. I don't know enough about LaTeX to know why it worked, but I'm content that it did. Thank-you so much!
